New Patient Registration

consultation1The doctors and staff welcome new patients who live within our practice area. 

We require new patients to register in person with1 proof of address (Utility Bill, Rent Agreement, Council Tax or Land Line Telephone Bill) and 1 photo identification such as a passport or photo drivers license.

You can register on  Monday,Tuesday, Wednesday, Thursday & Friday between the hours of 9.30 - 12.00 and Monday, Tuesday & Friday 15.00 - 17.30

Late registration is on a Wednesday between  15.00 - 18.45,

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of the registration form and new patient questionnaire which you may fill out and bring in with your proof of address and I/D. When you come to the surgery you will be asked to sign the forms to confirm that the details are correct.

Medical treatment is available from the date of registration; please contact reception for further information.

 You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
